The US has unwittingly committed the mistake of putting in place a Shia led government to rule over Sunnis in Iraq–ignoring that Sunnis and Shias have been blood enemies since the founding of Islam, almost 1400 years ago–this in turn has earned the enmity of Sunnis and enabled ISIS to rapidly spread across the world.
The natural empathy some Muslims feel with people of their faith–after witnessing their suffering in the media–hold the US responsible and feel emotionally compelled to take revenge, which has resulted in recent attacks in Orlando, San Bernardino, and Boston.
The US executive branch's belief that as a sovereign government it can do no wrong, has directly contributed to the rise of citizen militias–as articulated in the 2nd Amendment– which Constitutional Law scholars, such as Erwin Chemerinsky, have been saying for decades that the US judicial system is subservient to the executive branch.
Oklahoma City bomber Timothy McVeigh, and Stephen Paddock, responsible for the Las Vegas attack, were both one-man militias that rose up to punish the government for its sovereign belief that it can do no wrong.
The final root cause is the mental issues and/or alienation from society perceived by some people, which has caused some citizens to commit horrendous attacks in their misguided attempts to bring attention to their suffering.
