DALLAS and ALBANY, N.Y., March 10, 2015 /PRNewswire/ -- St. Peter's Health Partners Medical Associates, P.C. (SPHPMA), a multispecialty group with more than 350 physicians and advanced practitioners, has selected a comprehensive web-based population health management, care management and patient engagement platform from Phytel to help more patients across the community maintain and improve their health.
The physician group, which is directly affiliated with the Albany region's largest health system, St. Peter's Health Partners, expects the Phytel platform not only will enhance the care of patients but also help ease the group's transition from traditional fee-for-service billing to the increasingly common value-based model of reimbursement. Under value-based or "accountable care," providers are rewarded for the overall health of their patient population rather than the volume of services they provide.
"We believe Phytel will be a valuable tool to help manage our high volume of patients, toward the goal of improving the health of all people in New York's Capital Region," said Thomas Lawrence, M.D., chief medical officer of SPHPMA. "Phytel's automated processes allow us to engage patients who are most at-risk and partner with them in better managing their health, including those who are trending toward becoming sick. By automating this work we expect to help more patients while freeing staff to focus on quality care delivery."
The Phytel platform aggregates, reconciles, and normalizes patient data from a variety of sources, to match patients against evidence-based care guidelines. Phytel then identifies patients who have gaps in their recommended care and are at high risk of acute health episodes, and engages them through automated and personalized follow-up. SPHPMA will be able to easily generate reports to measure the group's quality performance on shared savings plans and other value-based arrangements from virtually any payer.
Dr. Lawrence said SPHPMA selected Phytel after a rigorous evaluation based largely on its success driving population health for similar-sized physician groups across the country.
"For physician groups and health systems who are engaging in Accountable Care Organizations (ACOs) and other forms of accountable care, nothing is more critical than ensuring they engage patients effectively," said Steve Schelhammer, CEO of Phytel. "With the assistance of our analytics tools, care coordination platform, expertise in patient engagement, and knowledge of population health management, we're confident that St. Peter's Health Partners Medical Associates will achieve their goals."
