Sense Health Aims to Enhance Care for High-Needs Medicaid Patients in the Bronx
Following a successful randomized control trial, Sense Health's technology will be rolled out for care managers supporting Medicaid patients in New York City
NEW YORK, Sept. 9, 2015 /PRNewswire/ -- Sense Health is expanding its role in Medicaid care management programs in New York, the second largest state Medicaid program. Sense Health's patient engagement technology enhances a care manager's ability to provide personalized, outcomes-driven support for growing patient caseloads through two-way messaging and care coordination features. As part of this expansion, Sense Health is proud to announce its collaboration with Montefiore Health System to build out Montefiore's mobile patient communication abilities.

The partnership was sparked following a successful randomized control trial within a Medicaid patient population at University Behavioral Associates (UBA), which is affiliated with the Montefiore Health System. In the trial, Sense Health demonstrated a significant improvement across key patient engagement metrics, including: self-reported patient adherence to appointments (40% increase), medications (12% increase) and care plan goals (7% increase), compared to patients receiving care management services alone.
