SAN FRANCISCO, July 13, 2016 /PRNewswire/ -- pMD, the innovation leader in health care technology, has added automated hospital discharge alerts to its mobile and web apps that significantly improves follow-up care for patients after they are discharged from the hospital. The real-time alerts also allow physician practices to increase their financial performance for scheduling timely follow-up appointments with patients.

pMD's mobile charge capture software, which allows doctors to record billing and lightweight clinical information into their phones at point of care, sends an automated alert to the patient's care team immediately after a patient is discharged. The alert includes pertinent information about the patient, including a discharge summary, medication changes, care coordination notes, or other follow-up information.
David Doane, Director of Reimbursement at Dallas Nephrology Associates, spearheaded the implementation of pMD's discharge alerts among his team of over 75 physicians.

Practices participating in Medicare's Transitional Care Management (TCM) can receive additional incentive funds by following up with their patients after they are discharged. In order to receive the payment, staff must contact the patient within 48 hours after discharge, and a physician must see the patient within two weeks. pMD's alerts automate the communication between the physician and scheduling staff, allowing them to follow up with the patient within 48 hours and receive the additional funds.
