BOSTON, Dec. 18, 2017 /PRNewswire/ -- PatientPing, a health technology company connecting providers to seamlessly coordinate patient care, today announced a new partnership with Mercy Health, a Catholic healthcare ministry serving Ohio and Kentucky, to enable real-time patient care coordination. With the PatientPing platform, Mercy Health can connect with both hospitals and post-acute care facilities to access and share data outside of their health system. This enables Mercy Health to support care transitions for their Medicare Advantage and Comprehensive Primary Care Plus (CPC+) patients, as well as those under their various bundled payment models. Mercy Health expects to cover approximately 200,000 lives with PatientPing's platform. The rollout will occur over the next several months before expanding to additional regions in Ohio and Kentucky.
