Miami Children's Hospital selects PatientPoint for Patient Engagement and Care Coordination Solutions
Top Pediatric Hospital leverages the HealthSync Care Coordination Platform to streamline administrative and clinical processes to achieve best in class patient engagement
ORLANDO, Fla., April 23, 2012 /PRNewswire/ -- PatientPoint™, a leading provider of care coordination and patient care management solutions, today announced that it has been selected by Miami Children's Hospital to provide consulting services and to implement its patented HealthSync Care Coordination Platform.
Miami Children's Hospital is a world leader in healthcare with over 650 physicians providing comprehensive services in over 40 pediatric specialties and sub-specialties. "We signed with PatientPoint in February and went live within 6 weeks with over 40 check-in points across the hospital and with complete integration to our new ADT systems. Based on the iOS platform, HealthSync offers best-in-class patient tracking, patient care coordination and a complete set of mobile applications for patient engagement. We believe it will provide a highly effective, end to end, enterprise-level solution for care coordination and patient engagement during pre-care, point of care and post care," said Ed Martinez, CIO of Miami Children's.
PatientPoint will deploy the HealthSync Care Coordination Platform to the Miami Children's physician network across multiple care settings. Data will be aggregated from Electronic Health Record registries, Pharmacy Benefits Managers, and patient self-reported information to identify and resolve clinical care gaps. Various patient engagement technologies, including wellness digital gaming and mobile and online applications will be leveraged for pre-and post-care reminders and to ensure medication and treatment adherence. In addition, tablet based check-in and check-out solutions will be used to simplify administrative processes, address medication and clinical gaps in care, and facilitate clinical surveys and screenings like ADHD. By providing the most relevant clinical intelligence at the right time, Miami Children's will be able to better align physicians and care teams with their patients, resulting in higher quality of care at significantly lower costs.
"When we first met with the leadership at Miami Children's Hospital and heard their vision of coordinated, patient-centered care, we felt that there were many synergies between our organizations and a great opportunity to partner and bring to market a patient engagement platform second to none," said Raj Toleti, CEO of PatientPoint. "We are truly fortunate to be partnering with this prestigious organization and committed to ensuring they are successful in achieving their strategic objectives."
