Landmark Health announces the enrollment of its 5,000th patient!
HUNTINGTON BEACH, Calif., April 4, 2016 /PRNewswire-USNewswire/ -- Landmark Health, an industry leader of home-based medical care for patients with complex health needs, announced the enrollment of its 5,000th patient today.
Landmark Health was founded in 2013 with the mission of delivering team-based medical care to complex and chronically ill populations. Landmark clinical teams are physician-led and interdisciplinary in nature—these teams deliver comprehensive medical care to patients in the comfort of their homes, and are available for in-person consults 24/7. Landmark's clinicians partner closely with community providers, including primary care, to deliver collaborative and coordinated medical care for patients, many of whom have challenges accessing medical care with the frequency that their clinical profile demands.
Landmark Health conducted its first home visit on October 6th, 2014. Since that inaugural visit, Landmark has expanded its operations to encompass five urban markets in four states, including New York, Washington, Oregon and California.
