WEST DES MOINES, Iowa, Nov. 18, 2014 /PRNewswire/ -- Extended Care Services of Iowa, Inc. (ECSI) in affiliation with the Iowa Health Care Association (IHCA) has selected COMS Interactive, LLC (also known as Clinical Outcomes Management Systems or COMS) as a Preferred Partner. ECSI enters into partnerships with a select group of product and service providers to offer innovative programs to IHCA members. The exclusive three-year Partner Agreement will focus on helping IHCA member facilities improve clinical and financial outcomes by deploying the COMS disease management product suite, Daylight IQ®.
IHCA and its assisted living arm, the Iowa Center for Assisted Living (ICAL), represent more than 700 Iowa Nursing Homes, Assisted Living/Residential Care Facilities, Home Health Agencies, Hospice, and Independent Living/Retirement communities throughout the State. Notably, IHCA members serve more than 42,000 Iowans in the long-term and post-acute care continuum.
"A primary goal of ECSI is to offer products and services to IHCA members that allow them to provide the highest quality of care," stated Steve Ackerson, Executive Director, Iowa Health Care Association and President, ECSI. "Education regarding the significant benefits of an advanced disease management program is key to extending our clinical expertise and we are pleased to introduce Daylight IQ® to the IHCA membership."
Daylight IQ® has been successfully deployed in Skilled Nursing and Assisted Living Facilities and Home Care organizations throughout the US. The Software as a Service (SaaS) product features a series of integrated, disease-based clinical protocols that significantly empower the entire nursing team at the point of care, while facilitating enhanced communication between all parties. A key element of Daylight IQ® is the reduction in unnecessary hospital readmissions by as much as 50%, a related decrease in premature mortality rates, and an increase in successful discharges.
"The stated mission of providing a supportive and ethical environment, based upon professional and compassionate care to the frail and elderly, is a testimonial to the caring thought leadership of the IHCA and ECSI teams," noted Edward J. Tromczynski, Chief Executive Officer, COMS Interactive. "The focus on providing advanced clinical products is further evidence of IHCA leadership's innovative approach to the care process."
Market research indicates that the average Nursing Facility, Assisted Living or Home Care resident has a complicated disease profile, with one primary disease and up to eight secondary diseases or conditions. Approximately 80% of re-hospitalizations are due to the worsening of secondary or new diseases. By providing early detection of even minor changes in condition, Daylight IQ® highlights potential problems and offers caregivers the opportunity to respond, preventing further progression of the illness.
