ALISO VIEJO, Calif., Nov. 13, 2018 /PRNewswire/ -- ConcertoHealth, the leading provider of specialized primary care and supporting clinical services for vulnerable, frail and elderly patients, announced today that it is joining forces with Regence BlueShield, and expanding its field-based care model to support the company's physician network in King, Pierce and Snohomish counties.
With the recent expansion of its field-based care model, ConcertoHealth now provides more support to health plans and their primary care provider (PCP) networks by caring for their highest-risk patients. The expansion in Washington state builds on the success of the ConcertoHealth care model, which dedicates field-based care teams and "wraparound" support services to primary care physicians. Rather than replace or compete with PCPs, ConcertoHealth supports their performance with an array of clinical resources, including social workers, care managers, pharmacists, and nurses to care for patients in any care setting, when they are most in need.

With preventive care as the focus, ConcertoHealth will help PCPs in the Regence Medicare Advantage (MA) network better engage their patients. ConcertoHealth patient outreach specialists will make contact by telephone to help patients schedule wellness visits with their PCPs before Dec. 31, 2018. If patients are unable to make it to their PCP's office, ConcertoHealth will send clinical professionals to visit patients in their homes or other care settings as needed.
In addition, PCPs in the Regence MA network will have access to patients' clinical histories through the ConcertoHealth Patient3D Provider Portal. This population health data technology provides a comprehensive, 360-degree view of patients' complete medical history and consolidated records, in real time and at the point of care. Patient3D empowers care providers and managers to make informed decisions based on actionable data and predictive algorithms with prompts to address gaps in care, leading to lower costs of care and improved health outcomes.
