SAN FRANCISCO, March 25, 2019 /PRNewswire/ -- ClearCare, Inc. today announced general availability of its Quality of Care module, further empowering home care agencies to improve outcomes and lower health care spending by monitoring, reporting, and preventing serious complications among patients with chronic conditions.
The Quality of Care module consists of two main components:
- Change in Condition protocol, developed as part of a multi-year study with Harvard Medical School, provides transparency into clients' health by identifying changes in a patient's condition and monitoring (or helping to manage) these changes in the home. Any changes in status are communicated in real-time to a care manager who reviews the reported change in condition and decides on the appropriate course of action.
- Hospitalization Tracking gives agencies visibility into client admissions providing for more seamless continuity of care and transitions back to the home. Tracking over time helps to identify trends, making it possible to take proactive action to reduce readmissions.
The Change in Condition protocol has been deployed and tested across over 150 thousand patients in the last four years in a Harvard Medical School study supported by Right at Home and ClearCare.
