Home is the unknown, underutilized setting of care that can help appropriately accelerate the transition from high cost fee-for-service models to value-based care. Not only do patients prefer to heal or age in the comfort of their home, but doing so also safely and effectively reduces the reliance on higher-cost settings, such as hospitals or skilled nursing facilities. The Centers for Medicare and Medicaid Services (CMS) and legislators are increasingly focused on care in the home, as evidenced by the Comprehensive Care for Joint Replacement model and proposed mandatory bundled payments for cardiac care.

For a patient with significant health conditions, such as chronic obstructive pulmonary disease (COPD) and congestive heart failure, these statistics potentially translate into dangerous gaps in care after a hospital stay.
With the real-time, continuous HomeBridge connection between patients, providers, centralized nurse teams, telehealth providers and community resources, the patient's entire care team will be able to see a daily record of information like blood pressure and heart rate, and intervene when needed.
