Care4Life combines the best in diabetes education with proven evidence-based behavior change techniques. The program uses text messages, a personal online web portal and a mobile website to connect, engage and support people living with diabetes.

Along with increases in medication/appointment attendance and weight loss, Care4Life participants are more knowledgeable, confident and effective in managing their diabetes.
