SALT LAKE CITY, May 15, 2015 /PRNewswire/ -- VorroHealth today announced that its KeyHIE™ Transform solution is one of a few select tools long-term and behavioral healthcare providers can use to qualify for $1 million in grant funds to help improve sharing of health information. The grant is available from the Office of the National Coordinator for Health Information Technology's Community Interoperability and Health Information Exchange (HIE) Program.
The ONC's official Funding Opportunity Announcement (FOA) specifically lists KeyHIE Transform because it is a proven, low-cost software tool that enables a nursing home or home health agency to contribute patient information to a Health Information Exchange (HIE) to provide better and more coordinated care. Patient assessment information is generated from a SNF's Minimum Data Set (MDS) system or from an HHA's Outcome and Assessment Information Set (OASIS) system and then converted into a Continuity of Care Document (CCD / C-CDA) containing the patient assessment information. The process is HIPAA and PHI compliant and works with or without an Electronic Health Record (EHR) system.
VorroHealth partnered with Geisinger in 2013 to establish KeyHIE Transform, developed under the Keystone Beacon Community Program. One of 17 communities selected by the U.S. Department of Health and Human Services to receive funding, Keystone Beacon is a network of healthcare providers in Central Pennsylvania — doctors, hospitals, long-term care communities and others — that joined together to help coordinate and manage their patients' care, reduce hospital admissions, and improve their patients' quality of life.
The Community Interoperability and HIE program will provide funds for up to 10 community organizations, state or local government agencies, or other community groups. The awards will help unlock health information and better integrate community resources advancing better care and healthier communities.
