'Transforming Chronic Care' aims to inform the sector and educate the wider public about the innovative ways of improving collaboration within the healthcare community, as well as of identifying those at high risk, preventing the onset of chronic conditions.
Introduced by Newscaster Johanna Botta, the program combines key industry interviews and news-style reports with sponsored editorial profiles uncovering cutting-edge products and services focusing on improving medicine practice with significant cost savings:
