ATLANTA, March 4, 2014 /PRNewswire/ -- Summit Health, a health system serving the population of south central Pennsylvania, today announced the successful implementation of Wellcentive's suite of solutions and services for population health management. Summit Health contracted with Wellcentive to implement a technology infrastructure that would enable its leaders and care providers to manage a wide variety of quality initiatives and risk-based contracts.
Summit Health implemented Wellcentive as part of its long-term strategic plan for managing risk across the care continuum. With the platform for population health management now live, the health system is prepared to participate in a variety of payer-sponsored quality initiatives. Wellcentive will enable Summit Health to manage multiple pay-for-performance and risk-based contracts, such as the Highmark® Quality Blue Physician Program. The health system is also preparing to participate in patient-centered medical home (PCMH) initiatives, including gaining certification through the Accreditation Association for Ambulatory Health Care.
"Few things are more important to us strategically right now than preparing to manage clinical and financial risk," explained Frank Mozdy, M.D., chief medical officer at Summit Health. "Choosing Wellcentive as our partner in this critical effort was a deliberate process that involved almost a year-long evaluation of numerous vendors. Our implementation process has confirmed that Wellcentive is the right partner for aggregating and integrating data from across the continuum and making it available to the clinicians and care managers who work every day with patients."
"Summit Health is leading the Pennsylvania market in establishing a complete infrastructure for population health management," said Tom Zajac, Wellcentive's CEO. "With our single, integrated platform, they are now prepared to meet the varying requirements of all of their value-based initiatives—now and in the future."
Wellcentive's platform enables healthcare organizations to aggregate data, standardize best practices for managing care, and continuously improve outcomes. It helps clinicians, administrators and staff identify high-risk patients, prioritize care-management efforts, close gaps in population care, and track, measure and report on quality and cost performance at an enterprise level. This functionality bridges the divide between patient visits and helps ensure that patients receive care in the lowest-cost, most-appropriate setting.
Summit Health's use of the Wellcentive platform will focus initially on managing high-risk populations for the following chronic conditions: diabetes, heart failure and chronic obstructive pulmonary disease (COPD). From a preventive standpoint, the health system will leverage the platform to promote adolescent and well child visits.
