SHP's Scorecards have always provided reliable and actionable insights to its customers, but the new Scorecard Report Suite delivers much more.

With the launch of the new Scorecard Suite, SHP customers can choose to utilize either Patient Source or Staff Performance reports. The Patient Source view provides a detailed and comprehensive look at all quality of patient care outcomes to share with referral sources. With changing reimbursement models, referral sources are facing numerous value-based initiatives – from ACOs to bundled payments. Home health agencies can now share results and open a dialogue by bringing SHP performance reports to key referral sources. With Primary Payer as a new breakout group, payer-based scorecards can be used to track and share agency performance under new risk-based insurance programs.
The Patient Source reports can also be run by primary diagnosis group for example "Hips and Knees". Under the Centers for Medicare and Medicaid (CMS) Bundled Payment for Care Initiative (BPCI) and Comprehensive Care for Joint Replacement (CJR) demonstrations, tracking quality for these specific diagnoses can encourage providers to work together to improve quality and coordination of care from initial hospitalization to the end of care. Post-acute care providers can use SHP Scorecard reports for these diagnoses to manage performance and share outcome results.
SHP's popular Staff Performance reports now deliver a greatly increased range of measures. Examples include a full range of hospitalization metrics and a new proprietary, composite quality score to easily organize agencies by overall performance in real-time and take action on those requiring improvement.
