DALLAS, July 16, 2015 /PRNewswire/ -- Premier Patient Health Care and Patient Physician Network, two physician owned and physician led organizations, recently agreed to terms forming an alliance known as Premier Patient Network (PPN). This alliance creates an advanced services organization combining the strengths of both companies centered on supporting clinically integrated, independent physicians in the provision of health care in a value-based environment. The cornerstone of value is optimal quality in both care and cost.

Premier Patient Network will enable its 882 partner physicians to adapt to a rapidly-changing healthcare market. As the system shifts from a volume-based, acute care model to a value-based, preventive care model, physicians are consistently required to do more with less. The new PPN supports physicians through an established, purpose-built value-based operating model powered in part by Premier's proprietary analytics tool, Premier 360. This SaaS based tool enables physicians to focus attention where it will most effectively improve patient care and impact overall wellness.
"We are excited about this alliance," said Anwar Kazi, CEO and Management Partner of Premier Patient Health Care. "In 2014, we used this operating model to achieve more than $9 million in shared savings. We look forward expanded opportunities for independent physicians who chose to participate," Kazi continued.
The new PPN offers a care delivery network unique within the Dallas Fort Worth community, providing technology and programs for population health, care coordination, predictive modeling, post-acute care and transitional care management. Unique attributes include:
- One home for multiple lines of business including: Medicare Shared Savings Program, commercial fee for service and ACO, and commercial Medicare and ancillary service lines
- Proprietary technology platform with data sharing and predictive modeling technology powered by Clinical Pathology Laboratory's iMorpheus that translates real-time data into powerful preventive road maps…helping physicians improve quality of care and meet pay for performance goals
- Chronic Care Management and Extender programs with additional people and process resources for help with complex cases
- Dedicated ancillary partnerships providing a seamless, coordinated experience for patients and physicians across the continuum of care.
"We are very pleased to partner with Premier to form this alliance. Independent physicians can now work smarter, not harder, positioning themselves and their patients at the center of care," said Murray Fox, MD, President & CEO of Patient Physician Network. Dr. Fox continued, "They are empowered to focus more on why they chose medicine as a career in the first place…building relationships with patients and improving the health and wellness of their patients and their community."
