ORLANDO, Fla., Sept. 16, 2013 /PRNewswire/ -- PatientPoint, Inc. today announced the release of PatientPoint Tracker, the next generation of patient engagement technology allowing hospitals and other providers to track patients across the care continuum, identifying gaps in care and opening the door to improved outcomes and patient satisfaction.

Through customized dashboards, hospitals can follow the patient from the time an appointment is scheduled through the check-in process and visits to one or more departments, continuing with post-care tracking, such as follow-up appointments. PatientPoint's innovative integrated approach helps hospitals operate more efficiently while enabling a better patient experience.
"Having multiple touchpoints makes it challenging to provide coordinated and efficient patient access and engagement," said Tom McGuinness, CEO, PatientPoint. "PatientPoint Tracker offers that next level of patient engagement with a detailed level of patient tracking, reporting and integration so providers can now optimize their care management workflow. Our integrated solution also helps support new care models, such as population health management, accountable care, risk-based contracting and bundled payments."
Because PatientPoint Tracker integrates with a hospital's admissions-discharge-transfer (ADT) system, there is minimal data entry from staff. PatientPoint Tracker is also integrated with the PatientPoint Care Coordination Platform to track patients across the care continuum.
Miami Children's Hospital is currently using PatientPoint Tracker to trace patients from the time they enter the patient access area through their visits to service units. The dashboard provides clear visibility to current patient wait times, and managers can use the software tool to adjust staff levels as necessary.
"We are now better able to integrate our patient flow and gather data points on the patient experience across the care continuum," said Ed Martinez, CIO of Miami Children's Hospital. "Since PatientPoint Tracker integrates with PatientPoint Care Coordination, we will better understand opportunities to further enhance outcomes. In addition, PatientPoint Tracker supplements PatientPoint's Outcomes Research Solutions (ORS) Platform to accelerate the recruitment and enrollment of clinical trial participants."
Noting that PatientPoint is committed to simplifying the complex healthcare system for both providers and patients, Raj Toleti, president of Coordinated Care Solutions and chief technology officer, said, "Our strategic technology solutions allow healthcare organizations to track, measure and manage key metrics, leading to better population health, improved outcomes and a lower cost of care."
