Made up of several modules, the application aims to connect care providers, patients and their families outside of the hospital setting. The announcement was made at HIMSS 2012, being held February 20–24 in Las Vegas.

SAP Collaborative E-Care Management is intended to allow for better management of health by offering individualized treatment plans and educational content while maintaining continuity of care. The solution aims to enable payers to help their members better manage health and wellness for more personal engagement and better compliance. In addition, self-insured organizations are planned to be able to take advantage of fully enabled wellness and productivity management programs for their employees, helping to reduce costs and improve satisfaction and care quality.
Designed to mobilize and fully enable wellness, post-discharge transitional care and chronic condition management programs, SAP Collaborative E-Care Management is planned to be accessible through a secure social network platform connecting care providers and supporters and facilitating collaboration in a closed online community. The collaborative framework is planned to have an open architecture, which aims to allow for working with existing applications and developing individualized business tools and integrations. The online community is intended to incorporate security mechanisms and controls to help ensure information is both highly secure and usable and shareable.
SAP Collaborative E-Care Management is also intended for use to analyze information with intuitive data exploration and visualization with healthcare-specific analytics. By leveraging proven points of engagement in the form of mobile devices and social media with personalized profiling, incentives and information, SAP aims to engage consumers in cost-efficient, safe and quality-based healthcare.
