Kryptiq Announces New Chief Population Health Officer, Jacquelyn Hunt, PharmD, MS
Leading Population Health Management Company Establishes Position Focused on Team-Based Care and Patient Engagement
BEAVERTON, Ore., June 18, 2014 /PRNewswire/ -- Kryptiq, a market leader in population health management, announced today the appointment of Jacquelyn Hunt, PharmD, MS, as its chief population health officer. In this role, Dr. Hunt will oversee development and delivery of Kryptiq CareManager™ population health management programs. In this capacity, Dr. Hunt will direct curation of evidence based medical guidelines with Kryptiq's 20-member Clinical Advisory Group, ensuring CareManager utilizes these guidelines and protocols to better inform providers at the point of care.

Most recently, Dr. Hunt served as the chief quality officer and chief information officer at Bellin Health System, an integrated healthcare delivery system and a Pioneer accountable care organization. Prior to her tenure at Bellin, Dr. Hunt completed a Merck Foundation Fellowship with the Institute of Healthcare Improvement focused on the Triple Aim, large-scale system transformation, and patient-centered health technology. Additionally, she was previously the executive director of quality and care redesign at Providence Health & Services, a not-for-profit healthcare ministry comprised of 32 hospitals, 350 physician clinics, senior services, supportive housing, and many other health and educational services.
Dr. Hunt received her B.S. in Pharmacy from Oregon State University with a certificate in gerontology. She earned a Doctorate in Pharmacy from the University of Texas Health Science Center where she also completed a two year residency in pharmacotherapy and ambulatory medicine. Dr. Hunt later received her M.S. in clinical research design and statistical analysis from the University of Michigan.
