ComplexCare Solutions, Inc. (CCS) Endorses CMS's Affirmation of the Clinical Value of In-home Assessments
New "best practices" align with CCS's current approach to connecting in-home assessment to high-touch, high value care management programs that improve member outcomes.
NEW YORK, March 9, 2015 /PRNewswire/ -- ComplexCare Solutions, Inc., a leading healthcare payer services company that specializes in providing face-to-face care management services and risk assessment for complex care needs populations, today announced support for the new CMS "Best Practices" that recommend utilizing in-home assessment to influence beneficiary care plans.
CCS has always used in-home assessment as an opportunity to develop NCQA compliant care plans and guide care management services to reduce cost and improve health outcomes. CCS in-home assessments examine clinical, behavioral, environmental and social determinants of health lead to specific recommendations for closing care gaps, especially those that can be readily closed such as fall prevention, medication reconciliation and nutrition assistance. To avoid the need for higher-cost facility-based services, CCS high-touch, high value care management programs use low-cost, community-based resources coordinated by multi-disciplinary care teams to help health plan members stay on course and achieve a greater level of independence. In fact, most of the members served by CCS "graduate" back to self-care within 4-6 months.
Jeff Emerson, CEO of ComplexCare Solutions said, "It never made sense to assess members in their home, build a relationship, and come away with diagnoses and information only for the purposes of risk adjustment. Our solutions are built around the principle that data driven tools like assessments play an integral role in developing and executing care plans that engage members in improving their own health. I envision significant growth opportunities for ComplexCare in response to this recent announcement by CMS."
