Care Improvement Plus Unveils 2012 Medicare Health Plans
Nation's largest chronic conditions special needs plan reaffirms commitment to providing affordable, high quality health care solutions
BALTIMORE, Oct. 3, 2011 /PRNewswire/ -- In preparation for the October 15th start of Medicare open enrollment, Care Improvement Plus—operator of the largest chronic conditions special needs plan(SNP) in the country— has unveiled its 2012 Medicare health plan benefits, reaffirming its commitment to provide affordable, high quality health care solutions to its members.

Care Improvement Plus' service area will include select counties in six new states in 2012: Illinois, Indiana, Iowa, New Mexico, New York, and Wisconsin. In addition, for the first time, Care Improvement Plus will offer its Dual Advantage (PPO) Special Needs Plan in select counties of Georgia and South Carolina for beneficiaries with both Medicare and full Medicaid coverage.
Across its service area, Care Improvement Plus 2012 Medicare health plan benefits and services may include:
- $0 premium plan options providing affordable comprehensive coverage
- Vision, dental, transportation, and Over-The-Counter benefits
- Care management program including nurse coaching and a 24-7 nurse hotline
- Free annual in-home visits with a licensed practitioner
- Personalized counseling sessions with plan pharmacists
- Assistance with accessing social support services
- An open access provider network with no referral required for Medicare-covered services
- $0 copays for important preventive care services
Consistent with Medicare industry changes, Care Improvement Plus is providing eligible Medicare beneficiaries in the Part D coverage gap a 50 percent discount on brand name prescriptions and a 14 percent discount on generic prescriptions through its 2012 plans.
Currently, Care Improvement Plus has more than 109,000 members and operates the largest chronic conditions special needs plans in the country, focused on those with diabetes and/or heart failure. Overall enrollment in the plan has increased by 26 percent from October of 2010.
